  • Primary Benefit: Having the symbol on your plate grants the same parking privileges as a placard, without having to hang anything from your rearview mirror.

  How to Apply: The Medical Certification

  To get a placard or plate, you must submit the Individual Disability / Hearing Impaired Application (Form 96-0104).

  • Doctor’s Signature Required: A licensed healthcare professional (Medical Doctor, Osteopath, Podiatrist, Chiropractor, Registered Nurse Practitioner, Physical Therapist, or Physician Assistant) must sign the medical certification section of the form.

  • No Substitutions: We cannot accept letters, prescriptions, or medical reports. Only the specific MVD form signed by your provider is valid.

  • Deaf/Hard of Hearing: For these specific plates or placards, the certification must be completed by a licensed physician or a certified audiologist.

  Placards vs. Plates: What’s the difference?

  • Disability Placards: These hang from your rearview mirror and are issued to the person, not the vehicle. You can move them between any car you are driving or riding in.

    • Permanent: Valid for 5 years.

    • Temporary: Valid for 6 months (re-certification is required if the disability continues).

  • Disability License Plates: These are attached to a specific vehicle registered or leased in your name. They provide the same parking privileges as a placard but are permanently mounted.

    • Note: Disability plates are not available for trailers.
